The City of Bloomington is seeking an individual to provide professional public health nursing services that promote maternal, child and family health for residents in the south Hennepin county community.
Essential Functions of the Position
Provides public health nursing services to individuals and families (e.g. prenatal support and education, postpartum and newborn cares, parenting, child health, growth and development, etc.).
Delivers long-term evidence based Healthy Families America home visiting program to families at risk for poor childhood outcomes.
Coordinates daily referrals and assignments with other family home visiting nurses.
Advises and teaches clients about navigating systems.
Participates in various clinical and community health promotion programs.
Assesses the health and psychosocial needs of individuals and families.
Makes appropriate referrals to other community resources.
Maintains effective public relations and communications with community agencies, organizations and health care facilities.
Maintains accurate and timely documentation of client services via computer documentation.
Regularly travels to and from clients' homes, community sites, etc.
Works effectively across cultures.
Reports to work in a public health emergency.
Performs other duties and responsibilities as assigned.
Baccalaureate degree in Nursing.
Current license as a Registered Nurse in the State of Minnesota.
Certification as a Public Health Nurse in the State of Minnesota.
One year recent experience in maternal child health nursing (e.g. family home visiting, maternity/prenatal, postpartum, or pediatric).
Valid driver's license, car insurance and dependable car.
Ability to communicate both orally and in writing with the public and clients in an effective manner.
Ability to maintain timely and accurate records, using computerized charting and other software programs.
Experience working effectively across cultures.
Experience working with families who are at high risk for poor health outcomes (e.g. low-socio-economic or mental health issues).
Ability to lift and transfer lightweight equipment.
Advanced degree or certification related to maternal child health work.
Experience providing home visits to families.
Experience providing prenatal, postpartum and newborn care.
Experience working with diverse cultures and interpreters.
Experience working with interdisciplinary teams.
Experience utilizing an evidence based curriculum.
Ability to speak a second language in addition to English that has a recognized presence in the community (Spanish, Somali, Vietnamese, etc.).
Experience teaching about health topics to individuals and groups.
