Advanced Practice Registered Nurses / Physician Assistants
Provides culturally excellent clinical care to patients. Diagnoses and treats acute, episodic, or chronic illnesses independently or as part of a healthcare team. Orders, performs, and interprets diagnostic tests such as lab work and x-rays and prescribes medication by performing the following duties.
Duties and Responsibilities include the following. Other duties may be assigned.
Provides direct medical care to patients for a minimum of 30 hours per week.*
Collaborates with other departments within the agency to ensure integration and continuity of care.*
Monitors compliance with the chronic care management program.*
Precepts students to expand exposure to other disciplines to the populations we serve.*
Remains current in the proper and culturally excellent treatment and prescribing practices of LGBTQ+ patients.*
Attends monthly health center meetings.*
Maintains proper licensure and board certification.*
Complies with oversight by collaborating physician and works within the midlevel licensure.*
Functions independently to perform appropriate health interventions to acute, critical, and chronically ill patients.*
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.*
Collaborates with multidisciplinary team members by making appropriate referrals.*
Demonstrates knowledge and understanding of appropriate health needs of gay, lesbian, bisexual and transgender patients and transgender health care protocols (WPATH).*
Treats and oversees medical requirements associated with weekly STD testing and treatment.*
Serves as an advocate for the needs of individuals impacted by HIV/AIDS and the LGBTQ+ community.*
Performs on-call duties on a rotational basis.*
Attends outside functions at the request of the president and chief executive officer.
Provides presentations to community partner organizations as requested.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems. Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, musical notes, etc.) in its most difficult phases. Ability to deal with a variety of abstract and concrete variables.
To perform this job successfully, an individual should have knowledge of Microsoft Word, email software, cloud storage software, and Electronic Health Records software.
Certificates and Licenses:
Pennsylvania Licensed Registered Nurse
National Family or Adult-Gerontology Primary Care Nurse Practitioner certification
Typical medical office equipment
Knowledge, Skills, and Other Abilities:
Infectious disease treatment experience preferred
Experience working with LGBTQ+ community highly desirable
Master's degree (MS) in Nursing; and two to four years related experience and/or training; or equivalent combination of education and experience.
