The PHN position involves public health nursing work to promote the health of individuals, families and populations using knowledge from nursing, social and public health sciences.
The Nurse-Family Partnership partners registered nurses with first-time, low-income moms. The nurses provide home visits during a woman's pregnancy and until her child turns two years old. This program allows the nurse to build trusting therapeutic relationships with her clients and provide long-term parenting support for new families. The nurse home visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP and of Jefferson County Public Health.
Essential duties of the Public Health Nurse are:
Visiting of clients and families primarily in the home, but also in an office or clinic setting, to assess health and psycho-social needs.
Employee will provide nursing services through health promotion and disease prevention education, assisting clients and their families to establish health, parenting and life course goals, referring clients to community resources/supports and facilitating health care access.
Work also involves conferring with various community based health and service agencies to promote department and interagency program coordination. Outreach to referral sources and client recruitment are essential duties when building caseload.
BSN and minimum one year related work experience in a clinical setting.
Related work experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
Excellent written and verbal skills and basic computer skills needed.
Home visiting experience preferred.
Bilingual strongly preferred (English and Spanish).
Must be licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact.
Must have valid driver's license, if you are from out of state, you must obtain a valid Colorado driver's license within 30 days of hire date and you must not have any major violations, nor DUI, DWI, DWAI conviction in the past three years. Travel throughout Jefferson County required; must have reliable transportation.
Must provide immunization record at time of hire or prove immunity to comply with departmental Communicable Disease Control Policy.
