This position in Jefferson County Public Health (JCPH) involves public health nursing work to promote and protect the health of individuals, families, and populations using knowledge from nursing, social, and public health sciences. Work involves evaluating clients in clinic to assess health and as indicated, psycho-social needs; intervene through clinical services (including family planning, sexually transmitted infections, immunizations, and communicable disease control); provide education to achieve health promotion, risk reduction and disease prevention; referral of clients to community resources/supports; and facilitate health care access. Work also involves conferring and collaborating with various community based health and service agencies to promote Department and interagency program coordination and implement population based projects.

Involves professional nursing work and case management in a clinic health care setting to include:
Health assessment of individuals and families across the lifespan.
Working as part of a nursing care team to deliver care in a culturally appropriate and client-centered manner to diverse populations.
Conduct care coordination and health management with clients in the areas of family planning, sexually transmitted infections, immunizations, and communicable diseases.
Review of client immunization records for accuracy, education of clients about vaccine effectiveness, safety, and potential reactions, assessment for vaccine appropriateness and health status, and to prepare, measure, and administer immunizations to children and adults per Centers for Disease Control and Prevention (CDC) guidelines and medical director's standing orders.
Interviewing clients to obtain personal and routine health data in accordance with established policies and procedures for reportable communicable disease surveillance and intervention.
Educating on preventive health practices, illness/disease management, and counseling on high risk behaviors.
Documentation and communication of client care through maintaining accurate, complete, and timely medical records.
Participation in JCPH emergency preparedness trainings and exercises.
Collaboration and partnership with other local and state health agencies and community stakeholders.
Program outreach in the community as needed.
Participation in program planning and evaluation, ongoing relevant professional education, leadership as needed, assessments for policy and procedure development as needed, and to precept and/or mentor nursing students and new employees.
Represent JCPH and the public health nursing discipline to promote its concepts including health equity and social determinants of health.
Bachelor's degree in nursing and minimum one-year work related experience or equivalent.
Licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact at time of employment.
Current CPR Certification at hire
Strongly prefer experience in public health nursing, including family planning, sexually transmitted infection, communicable disease control, and/or immunization experience.
Experience in use of Electronic Health Records and MS Office highly preferred.
Spanish speaking ability preferred, but not required.
