Serve as a nurse care coordinator for families and children, birth to age 21, in the HCP Program for Children and Youth with Special Health Care Needs and maintain an active caseload per program requirements.
Provide family-centered, comprehensive, equitable, culturally sensitive and responsive care for children and families.
Conduct a comprehensive assessment and develop a shared plan of care in collaboration with children and families.
Equip children and families in gaining knowledge and skills to access services and support within the community.
Connect children and families to community resources which include, but are not limited to: medical and therapy services, respite care and parent support groups.
Communicate and coordinate child and family needs among community and health service providers.
Build and maintain productive working relationships with key community partners who serve families of children with special health care needs.
Serve as a liaison between families and systems of care. Communicate needs of families and advocate on their behalf to health and community service partners.
Inform community partners on the medical home approach to care for children and youth with special health care needs.
Adhere to HCP program expectations and reporting requirements.
Monitor policies, services and resources related to the health and well-being of children and youth with special health care needs.
Participate in local, state and community boards and other community outreach activities to promote services for children and youth with special health care needs.
Coordinate program activities with other Community Health Services program, JCPH divisions, partner agencies, service providers and community groups.
Spanish Speaking strongly preferred.
One or more years of work related experience
Community or public health related work experience in the areas of prevention and health promotion.
Experience working with children and youth ages 0-21 years and their families with
Experience working with children and youth ages 0-21 years and their families who have, or are at risk for having, developmental, behavioral, emotional or physical conditions.
Ability to develop trusting relationships with families and elevate their voices to influence and strengthen systems of care for children and youth.
Ability to collaborate within the community and interagency multi-disciplinary teams to foster a coordinated system of care.
Ability to represent JCPH and public health to promote health equity
Must be licensed as a Registered Nurse (RN) in Colorado, or in a state that has enacted the RN Nurse Licensure Compact.
Must have valid driver's license, if you are from out of state, you must obtain a valid Colorado driver's license within 30 days of hire date and you must not have any major violations, nor DUI, DWI, DWAI conviction in the past three years.
Must provide immunization record at time of hire or prove immunity to comply with departmental Communicable Disease Control Policy.
