Horizon Home Care and Hospice, located in Milwaukee, WI, has partnered with Froedtert Hospital to offer a unique, award-winning "Virtual Hospice" program and much-needed service for patients! Don't let "virtual" fool you into thinking this is done over Skype. You are at the bedside. This is your opportunity to create a positive hospice experience for patients and families within Froedtert Hospital. The Patient Care Coordinator assesses, develops care plans, and case manages hospital patients who will benefit from the support of Horizon hospice.
With the assistance of the Froedtert staff, you will ensure that patients receive the full scope of services available. Furthermore, the Patient Care Coordinator cultivates relationships with hospital physicians, nurses, social workers, and staff to educate them about hospice and how to identify patients who may be appropriate for this care. As this person is "the face of Horizon" at our sister hospital, we need someone with charisma and exceptional customer service. An outgoing persona and strong communication skills are essential.
Must be a self-starter able to perform with minimal supervision. Work independently, but collaborate with a variety of personnel.
3 night shifts per week from 6:00pm - 6:30am, including every other weekend.

Qualifications include a degree from an Accredited School of Nursing, a WI RN license, and at least one year of related experience.
