DIRECTOR INPATIENT CARE COORDINATION:
Responsible for strategic planning and implementation of inpatient clinical care coordination activities throughout the health system. Key functions include strategic planning, organizing and operational management of Clinical staff and managers. Participate in Transitions of Care Council established to triage and coordinate health system priorities (Managed Care / PHO strategies and contractual commitments). Collaborate with the Director of Ambulatory Care Coordination, General Manager of TPEC/Institutes, and General Manager of Navigator Transitional Care, ensuring the various entities are aligned and activities are coordinated. Develop and monitor key metrics, provide support and resources required and report results as requested.
MINIMUM REQUIREMENTS: EDUCATION: Master's Degree Nursing, Other Nurse Executive, Clinical Nurse Leader, or Care Coordination Certification within two years of assuming position or within 6 months of becoming eligible for the exam (whichever comes first)
EXPERIENCE: 4-5 years Clinical Care Coordination
