The Manager of Nursing Programs will provide leadership, administrative, and clinical support to Mary’s Center nursing programs. Overall responsibilities include supervision of Population Health/Resource Medical Assistants and Centering Coordinator, collaboration with clinical staff in planning, implementation, management, and evaluation of nursing programs and processes, care coordination of participants enrolled in nursing programs, facilitation of patient care groups, and nursing coverage in clinics when needed.
This position will report directly to the Director of Nursing Education and Programs.
Essential Duties and Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Supervise Population Health/Resource Medical Assistants.
Supervise Centering Coordinator
Work with Program Coordinators, Director of Nursing Education and Programs, and Chief Nursing Officer in writing, researching, and editing of key documents (e.g. reports, proposals, and presentations).
Work with the care team to create and/or edit educational and promotional materials for nursing programs.
Develop and implement protocols to support and promote nursing programs.
Facilitate meetings and conference calls related to nursing programs.
Coordinate activities necessary to achieve and maintain the quality and sustainability of nursing programs within the clinical practice setting.
Interact with and be accessible to different stakeholders, ensuring information sharing and communication between groups.
Train clinic staff in processes for nursing programs.
Work with an interdisciplinary team to create processes for identification, enrollment, and care of participants in care coordination programs.
Coordinate with care teams to recruit and evaluate appropriate candidates to care coordination programs.
Enroll new participants in care coordination programs.
Develop and provide follow-up on care plans for patients in care coordination programs.
Meet with participants in care coordination programs on an individual basis via phone and in-person as needed to support participant care.
Provide resources and support services for participants identified for care coordination programs who experience barriers to care.
Lead the Centering Steering Committee in the development, implementation, and evaluation of processes related to CenteringParenting and CenteringPregnancy.
Co-facilitate group visits – provide patients with health education and assistance with self-assessment activities.
Provide nursing care to group care participants via counseling, education, and/or referrals.
Collaborate with care teams to develop and refine the group care curriculum.
Provide nursing coverage in the clinic or via telephone as needed
Assists Nurse Manager in ensuring efficiency and proper use of appointments in the provider schedules.
Assists Nurse Manager and Director of Operations in coordinating patient flow, ensuring clinic efficiency.
Fosters communication and collaboration among members of the clinical team.
Plays an active role in developing and implementing a patient’s plan of care.
Provides patients with necessary health education and discharge instructions, whether in person or via telephone.
Assists the Medical Providers in refilling patient prescriptions and obtaining prior authorizations.
Collaborates with the Medical Assistants and Medical Providers to provide patients with lab and imaging results, as well as respond to medical requests by patients and outside entities.
Collaborates with the Medical Assistants and Medical Providers to complete any forms or paperwork deemed necessary by patients, schools, or healthcare entities.
Provides support for the Medical Provider, while functioning within the scope of a licensed registered nurse, in order to ensure optimal patient care.
Collaborates with referral specialists in specialty referral, including at times generating referrals per the request of a Medical Provider.

Education and/or Experience
At least 1 year of relevant experience. 2 or more years’ work experience preferred.
Registered Nursing licensure required in DC and Maryland. Bachelor’s Degree preferred.
Strong bilingual (English/Spanish) communication skills, both oral and written.
Thorough understanding of patient/client confidentiality issues and HIPAA compliance/regulations.
Experience working with culturally diverse and underserved populations. Awareness and sensitivity of sociocultural issues related to immigration. Demonstrated interest in public health, health education, or related field.
Strong project management capabilities required.
Proven leadership experience
Ability to multi-task, prioritize and adjust activities appropriately. Excellent organizational skills and attention to detail. Proactive and able to work independently and solve problems as needed. Team player with the ability to work collaboratively with staff that works remotely.
Advanced computer skills including experience with Microsoft Office (Word, Excel, PowerPoint, etc) and familiarity with creating and/or using charts, spreadsheets and databases a plus.
Physical Demands – Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.
Work Environment – Mostly in a typical office and/or home setting with a quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
