The CHS Registered Nurse will provide clinical services as part of the Camden Healthy Start project to prenatal and postpartum women at high risk of poor birth outcomes. Collaborate with local clinical providers. Jointly manage the provision of health services of high-risk women with Family Support Specialist case management and care coordination component. Implement strategies to manage risk with respect to the identification of emergent medical and psychosocial needs of clients. Interface with Camden Citys clinical providers on services to women at high risk of poor birth outcomes.
Performs Registered Nurse duties per the New Jersey Board of Nursing regulations
Develops and implements clinical protocols for the management of high-risk populations (i.e., high-risk OB/GYN with African American women, women with pre-eclampsia, previous PTB or LBW, obesity, previous PIH, gestational diabetes, smokers or late term enrollment into prenatal care).
Develops and coordinates the referral process via interfacing with local clinical providers to validate content of referral and identify emergent medical and psychosocial needs.
Works collaboratively with Family Support Specialists to develop a plan of care to address medical and psychosocial needs.
Builds linkages between the professionals involved in perinatal services and other community resources of value to mothers, fathers and infants.
Monitors high-risk clients progress in meeting goals and objectives.
Provides nursing services in varied venues to include the clients home, local providers offices or other environments to meet the clients needs.
Provides staff development training to increase staff skills and effectiveness related to perinatal topics.
Prepares regular progress reports and other reports as required by SNJPC or funding agencies.
Monitors clinical service performance and adjusts efforts as necessary

Registered Nurse licensed in the state of New Jersey
2 years of experience working with high-risk maternal and child health clients
Bilingual English/Spanish preferred
Must be culturally competent and have experience working with vulnerable populations
Expert knowledge of Camden Citys medical, social & mental health service systems.
Ability to plan strategically and complete tasks on time
Demonstrated leadership skills
Must be able to see to read documents and hear well enough to communicate with others
Must be able to write, type on a computer, and use a telephone
Must be able to operate a motor vehicle
Must be able to lift up to 10 pounds
