To supervise nursing staff who provide skilled nursing care to patients referred to the Home Health program in keeping with the Little Sisters of the Assumption (LSA) Mission that reflects: application of current nursing theory and standards of care, knowledge of program policies and procedures, following a physician's plan of care/treatment.
Supervises assigned staff in all appropriate aspects of care, ensuring the quality of nursing services. -
Oversee appropriate assignment of agency staff.
Supervises and reviews documentation and maintenance of patient records.
Ensure interdisciplinary coordination of home health services.
Evaluates the clinical competence of nursing personnel and completes annual performance appraisals which include a supervisory home visit.
Maintains professional skills and contributes to the knowledge base of nursing staff by attending workshops, trainings, and seminars.
Participates in orienting new staff, students and volunteers
Helps plan and participates in staff in-service education.
Coordinates and assists with reporting for special programs that include Maternity and Early Childhood Outreach, the asthma and environmental programs, and Haven's Relief (emergency financial relief)
Provides direct skilled nursing care to patients as needed.
Participates in the Agency Performance Improvement program, Professional Advisory and Utilization Review Committees.
Represents the Agency on professional and community committees and research projects as requested.
Manages the day to day operation of the Agency in the absence of the Administrator.
Other duties as requested.
Current license to practice as a registered professional nurse in New York State. Baccalaureate degree in nursing from an approved program.
One to two years of experience working in a supervisory position in a community health setting.
Must be able to establish and maintain successful working relationships with Home Health and Agency staff and the community. Bilingual English/Spanish a plus.
This is a full-time position, with a Monday through Friday work schedule (9am to 5pm). Some occasional evening meetings may be required, as well as, periodic weekend coverage rotation.
