The Cancer Patient Navigator is a Professional Nurse, with a specific body of knowledge regarding the diagnosis and treatment of cancer. This Nurse primary responsibility is to navigate a Cancer Patient through the Healthcare system at SPHN and to facilitate timely diagnosis to treatment through-put. The Cancer Patient Navigator demonstrates competencies in the diagnosis and treatment of cancer, assessment of patient and family educational and emotional needs, and the coordination of services. The Cancer Patient Navigator collaborates with the Medical Staff to provide individualized care to their patient. This individual is also responsible for collecting outcome data related to the program and evaluating the effectiveness of the program.
Graduate of an accredited school of nursing.
Current state registration or eligibility to do so.
Current BLS for health care providers
5 years clinical experience
Breast Health Educator or Navigator Experience
Certification, ONC or Breast Cancer Navigator Certification
