Responsible for coordination of care for all Total Hip and Knee surgeries. Manages Hip/Knee care coordination throughout the continuum of care and serves as a clinical intake coordinator for all Hip/Knee referrals that will include pre-op rehabilitation, pre-op registration of patients, pre-op testing, pre-op education specific to the patientï¿½s condition, and referral to other specialties for pre-op clearance.Demonstrates an understanding of the clinical pathway and educates patient and primary caregivers regarding plan of care preoperative, intraoperative, and postoperatively.Will serve as a resource for patient and primary care giver preoperative, intraoperative, postoperative, and post discharge from hospital.Organizes all education seminars and support groups.Responsible for the preparation for the disease specific Joint Commission certification and all on-going development of the program through maintenance of certification and performance improvements including quarterly meetings with the core team to discuss the status of the program.Responsible for MD, office staff and community marketing
Minimum Education: RN Licensure
Minimum Experience: 4 years RN(OR, Med-Surg., or previous Coordinator experience preferred)
