Coordinates nutrition/health education program within CHRISTUS GCTX departments; CHRISTUS Point of Light Clinic and CHRISTUS St. Mary's Clinic. Provide 1 days of service with CHRISTUS Point of Light and spend 4 days with CHRISTUS ST. Mary's Clinic to include CHRISTUS School Based Clinic. Give guidance and training to all GCTX providers/associates and work projects as assigned.
Plan and present programs at St. Mary's and Point of Light Clinics.
Group and/or individual nutrition lessons
Focus on a variety of health issues paying close attention to Diabetes and Heart Disease
Develop lesson plans and presentations in conjunction with providers/registered nurses
Assist in developing nutrition policies and procedures
Monitor patient's status and take appropriate actions involving provider/nurse
Develop material that takes into account patient diversity, culture, and education levels
Create and maintain documentation during patient/associate reviews for nutritional needs
Plan and present programs for CHRISTUS School Based Clinic(s)
Assist the nurse and medical assistants in developing healthy eating programs to share with their schools
Present Nutrition presentations to students and staff/faculty
Leads and participates in special school projects, e.g. Bodies in Motion
Works closely with other CHRISTUS associates as part of the health care team serving the patient
Maintains strict confidentiality
Follow CHRISTUS Physician's Group guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to protect or detect unauthorized disclosure of Protected Health Information (PHI).
Uses oral and written communication skills to effectively convey ideas in a clear positive manner that is consistent with the CHRISTUS mission.
Maintains established CHRISTUS Physician Group policies, procedures, objectives for quality assurance, safety, environmental and infection control.
Implements job responsibilities in a manner that is consistent with the CHRISTUS Mission and Code of Ethics and supportive of CHRISTUS Physician Group's cultural diversity objectives.
Performs other related work as required.
Graduate of an accredited Nutrition Program. Ability to read, write, and speak Spanish is required. American Diabetic Association registry eligible or registered.
Strong communication skills, includes speaking and written skills. Ability to network and collaborate with outside resources and service programs. Knowledge of community support systems. Ability to operate telephone, computer, copier, fax, and calculator.
