You Will Be Responsible For:
Completes a social history / assessment for each patient within forty-eight hours of admission.
The Plan of Care is coordinated with peers and other disciplines, as well as seeking out appropriate resources for implementing the plan of care as demonstrated through observation and peer review.
Completes the appropriate sections of the Resident Assessment Instrument, for each patient, by the fifth and fourteenth day after admission.
Makes appropriate and timely referrals to outside agencies.
Assists the Activities Coordinator as needed.
Documents the patient’s response to social service interventions.
Qualifications
What You Will Need:
A Bachelor Degree in Social Work or a Bachelor Degree in a human services field, including but not limited to; Sociology, Special Education, Rehabilitation, Rehabilitation Counseling, or Psychology.
One year of supervised social work experience in a health care setting, working directly with patients or residents.
BLS-A (Heart Saver Current)
Job Summary:
The Social Service Coordinator is a member of the Interdisciplinary Care Team. He/she is responsible for assisting the patient attain or maintain the highest practical mental, physical and psychosocial wellbeing possible. Social service needs are identified with the patient and family; formulates a plan, sets goals and educates the patient and family. Evaluates patient outcomes in response to the plan and revises as patient needs / responses requires. Represents the Transitional Care Unit in a professional and courteous manner.
