The Registered Nurse Case Manager practices within Outpatient Mental Health at VA Salt Lake City Health Care System. The Registered Nurse Case Manager will provide assessment and intervention services for patients enrolled in Outpatient Mental Health. Duties include but are not limited to the following: Provide psychosocial support, assist with problem solving and development of coping skills, and support the Veteran's personal goals for recovery. Obtain a health history of referred patients; data collection includes lifestyle patterns, pain issues, sleep/activity levels, support systems, cultural, social and ethnic factors, patient treatment preferences and goals. Administer long-acting injectable medications, including antipsychotics and opiate antagonists. Administer screening tools, including PHQ9, GAD, PCL, MOCA, Suicide Risk Assessment, and Audit C. Monitor ongoing treatments for adherence, medication side effects, and clinical outcomes, including worsening of symptoms, impairment and therapeutic response, and provide and/or coordinate appropriate follow up. Assess patients and provide additional support between visits with licensed independent providers (LIPs). Refer to, consult with and coordinate care with other VA health care providers. Serve as liaison for Veterans in community hospitals and treatment settings to facilitate continuity of care. Coordinate with other agencies and make referrals to provide patients with necessary community services. Function as an advocate for Veteran self-determination. Provide Veterans and their family members with education regarding their illness, medications and treatment plan. Facilitate psycho-educational and support groups. Actively participate in team meetings and other MH meetings as appropriate. Assist with education and act as preceptor for others who provide nursing care, i.e. nursing students, new hires, and Peer Support Specialists. Document mental health treatment within prescribed guidelines in CPRS. Provide coverage as needed for other staff in Outpatient Mental Health, and for other Registered Nurses throughout the OPMH Service. Other duties as assigned. Work Schedule: Monday- Friday 7:30 am to 4:30 pm Telework Authorized: Telework to be negotiated with supervisor based on program and agency needs. Financial Disclosure Report: Not required
