M Health Fairview is looking for LPN'sto perform COVID vaccinations for community clinics at multiple vaccination sites including schools, pop-up sites throughout the greater Twin Cities area. This position requires flexibility to work multiple shifts, demonstrate aseptic technique, injection site marking and safe delivery of vaccinations/ immunizations to patients as well as review patient consent forms to determine eligibility of patient to receive COVID vaccine. Must possess critical thinking skills and clinical assessment skills to handle possible emergency situations and also demonstrate strong customer service and problem solving skills. This is a temporary, benefits eligible position.

The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.

Collects data for patient assessments.

• Involves the patient, family, and other health team members in gathering relevant data.
• Documents data collected in the medical record.
• Reports unusual data or changes in patient condition to the RN, or physician/authorized practitioner in a timely manner.

Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient population, or individual patient.

• Follows the patients plan of care.
• Prioritizes work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner.
• Performs high and low risk technical procedures, including medications, defined by the clinical setting.
• Recognizes when different approaches may be needed based on age or cultural differences.
• Implements assigned aspects of the patient teaching plan.
• Documents care in the medical record.
• Effectively communicates compliance and concerns to delegating RN or physician/authorized practitioner.
• Takes action to assure continuity of care for all assigned patients.
• Adheres to all relevant policies, procedures, and nursing practice guidelines.
• Responds appropriately to emergent situations.

Communicates effectively with other members of the health care team.

• Documents care in the medical record.
• Communicates verbal and written information in a clear, concise, accurate, and timely manner.
• Uses communication approaches appropriate to patient/family needs.

• Vocational/Technical Training or Associate Degree
• Current Minnesota LPN License

• One to Three Years experience
