JOB SUMMARY: The Manager, Patient Care Services with the Hospital at Home Department , is a leadership role that has comprehensive operational authority and responsibility for designated clinical area(s). Their responsibilities include: fiscal accountability, maintaining quality and regulatory standards, maintaining a high level of patient/family centered care and service, patient and staff satisfaction, program development, managing and developing teams, and ensuring integration with the clinical enterprise. Provides feedback, coaching and mentoring to less experienced peers and other members of the health care team.
Hospital at Home Department
The newly developed Hospital at Home department focuses on providing hospital acute care in the home environment. Patients will be evaluated in the hospital setting and admitted to services, but deemed appropriate to be cared for in the home.
Oversee daily operation and clinical oversight of the Hospital at Home Department. Responsibilities include managing a group of nurses caring for patients in the home setting, a group of nurses in a "command center" for telemonitoring patients and triaging phone calls and care manager/intake nurses who will do the initial assessment and "admission" for patients considered appropriate for Vanderbilt Hospital at Home care.
