The LPN is a member of the health care team and functions with a high degree of independence, is self-directed, displays initiative and good judgement. Home Based Primary Care duties: Assists the HBPC RN with preparing medication boxes in the home Performs phlebotomy and specimen collection in the home Maintains adequate par levels of medical supplies and ensures all outdated supplies are removed from the supply closet Assists HBPC Administrative Staff with telephone coverage Assists HBPC Program Manager with QI projects and data collection through HBPC chart review Assists HBPC Administrative Staff with sixteen day tracking list for HBPC patients in acute care and skilled nursing facilities and performs follow-up calls to obtain weekly patient status updates Assists HBPC Referral RNs during periods of high workload Assists with HBPC Telehealth visits such as CVT, Video on Demand Incumbent will perform other duties as assigned. Travel and relocation expenses are not authorized. This position is in the Excepted Service. Incomplete applications will not be considered.
