The Licensed Practical Nurse (LPN) works under the direction of the clinic registered nurse (RN) or provider and reports to the specialty clinic manager. The LPN is a member of the larger Team. The LPN provides effective education to patients and/or family members in relation to common disease processes, perioperative instruction, medication, and/or prescribed regimes. The LPN demonstrates active collaboration and communication with the providers in clinic 1. Makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing the provider of changes in the patient's condition requiring higher-level intervention. 2. The LPN performs hands on patient care, and reports and records assigned nursing care in the Specialty Clinic including, but not limited to: a. Obtaining vital signs. b. Delegated procedures and treatments, including but not limited to: EKG, wound care, bandage application, cast removal, suture removal, medication administration, oxygen administration, blood glucose checks, doppler, pulse oximetry, weights, and other procedures requested by provider within the LPN scope of practice. c. Data collection as assigned. d. Provide peri-operative instructions. e. Clinical reminders and alerts as indicated. f. Check expiration dates of meds and supplies. g. Medication administration to include IV medication if required. h. Electronic record documentation, including documentation of procedures, medication administration and effectiveness, clinical information obtained during patient triaging, vital signs, clinical reminders, and other pertinent clinical information . i. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure. j. Completion of tasks designated by policies/procedures/protocols and/or assigned by supervisor, including but not limited to: pending access lists, EARR reports, and other assigned tasks/projects. k. Places patients in exam room in a timely and efficient manner using correct patient identifiers. l. Keeps exam rooms stocked with appropriate equipment/supplies needed for patient care. 3. Adheres to standards of practice and safe technique in preparation and administration of medication. 4. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate. 5. Based on the patient's goals, provides patient and family health education, to include self management, prevention and wellness. 6. Interacts and communicates appropriately with patients and family members. 7. Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills. 8. Adheres to principles of infection control, safety, and universal precautions. 9. Incumbent possesses knowledge of proper handling and pre-cleaning of reusable medical equipment. 10. Demonstrates competence utilizing special equipment needed to perform job duties. 11. Follows policy and procedures related to fire and disaster drills, safety and security measures. 12. Demonstrates initiative in assisting the Teamlet and larger Teams. 13. Assists in the orientation and competency validation of students, peers and clerical staff. Maintains a work environment that honors diversity. 14. Serves as a resource and advocate in the protection of patient and family rights. 15. Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles. 16. Promotes patient and personnel safety.
