This RN position is located in Nursing/Geriatrics at the West Haven, CT location. This position is full-time at 40 hours per week. The Community Living Center RN is part of a rehab, short term, skilled nursing team with a hospice wing. The RN is part of an interdisciplinary healthcare team and responsible for the medication administration, assessment, and physical and social activities of the residents on the unit. The RN will assess, document and observe patients progress and report any deviation from baseline to the clinicians. The RN is responsible for developing the patient care plan. The RN recognizes obvious signs and symptoms of changes in patients needs, and follows policies and protocols for the unit and the hospital. The RN will educate the family and resident, and follows ANA Nursing Practice Standards. The RN adheres to safety and health standards, regulations, work practices and follows infection prevention and environment of care safety measures.
