The Registered Nurse Community Care Coordinator is responsible for the oversight of non-VA episodes of care. This includes but is not limited to activities necessary to purchase patient care in the community in a manner that successfully integrates the community care with past and future VA care. The Registered Nurse Community Care Coordinator works collaboratively with all clinical divisions internal and external to the NCHCS medical center; in addition, he/she leads and works collaboratively with the administrative staff within the Community Care Program to ensure coordinated, timely, cost-efficient care to the Veteran. The major duties and responsibilities include, but are not limited to: Coordinate the process of care with the other team members to maximize efficiency. Document all patient care activities in a timely manner (per facility policy). Identify areas for continuing education and participate in in-service training (including staying current with all required education). Participate in developing and implementing the performance improvement plan (including quality monitors). Serve as a role model in the effective operation of an interdisciplinary team. Coordinates care for the patient with the VA referring provider and the community provider. Work Schedule: 8:00am to 4:30pm, Monday through Friday NOTE: Shifts may vary depending on patient care needs. Financial Disclosure Report: Not required
