ChristianaCare is currently looking for a Family Psychiatric Nurse Practitioner or Psychiatric Mental Health Nurse Practitioner to function as a member of a clinical team through collaboration with and a conduit to Behavioral Health, providing consultation, assessment, treatment, and behavioral health education within an integrated model of care. This role will be collaborating with primary and specialty providers in an outpatient setting as a psych prescriber. The selected candidate will perform a hybrid model of care with seeing patients in the practice as well as seeing patients virtually. This is a M-F position with no weekends and no holidays.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Demonstrates skills and knowledge necessary to provide care appropriate to patient population. Including the ability to obtain and interpret information, to identify diagnoses, to identify age and culturally specific needs, and to provide the appropriate treatment.
- Demonstrates the ability to provide highly skilled psychopharmacology prescribing consultation to the clinical team.
- Performs thorough and accurate psychiatric assessments on patients requiring psychiatric consultation.
- Provide direct patient care as directed by the Chair of Psychiatry, Director of Integrated Care, and supervising psychiatrist at the assigned primary care practice offices.
- Collaborates with interdisciplinary team, outpatient practices, and community partners as appropriate.
- Performs coordination with patient, medical staff, nursing staff, clinical team, and other embedded/integrated specialties
- Coordinates transition of care to community setting and/or internally as necessary; maintains knowledge of resources including community providers and support organizations appropriate for the patient population served.
- Documents in a timely, accurate and concise manner, using proper terminology, on department and CCHS forms, adheres to established guidelines.
- Utilizes teaching/learning principles coupled with an expert clinical knowledge base to provide education to patients and their families.
- Contributes to the development of staff by providing consultation, psychoeducation and suggesting best practice alternatives as appropriate.
- Works collaboratively with third party payors, Managed Care companies or other contracted concerns to ensure that quality patient care is maintained and managed with effectiveness and efficiency.
- Participates in Performance Improvement activities for the department, actively seeking to promote optimum patient care through participation in designated activities for improved performance, outcome measures and patient satisfaction.
- Demonstrates responsibility in maintaining established productivity levels.
- Demonstrates knowledge of patient's rights and maintains confidentiality of departmental and patient information.
- Demonstrates compliance with Christiana Care Health Service organizational and departmental policies and procedures and mandatory education requirements including but not limited to PEAT, safety, infection control, attendance, and dress code.
- Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
- Performs other related duties as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Master's Degree in Nursing
- Licensure or license eligible for State of Delaware
- Preferred multi-state licensure upon start or expected multi-state licensure acquisition within first year
- Board Certified as a Family Psychiatric NP/Psychiatric Mental Health NP
- Minimum of 1-3 years' experience working in a behavioral health setting.

- BLS certification
