Summary The Maternal and Child Home Visitation Nurse will be working in the Division of Pediatrics-Public Health on a new, 4-year prevention grant received from the state of Texas (upSTART Program). The Registered Nurse II will be implementing the evidence-based Family Connects home visiting protocol through conducting home visits to families and newborns in the Houston region. The Maternal and Child Home Visitation Nurse will assess families in 12 domains of physical and psychosocial well-being, including health and physical assessment of postpartum mothers and newborn babies in their home, assessing risk factors for the family, establishing a family support plan, assisting families in connecting to community agencies and appointments, and responding t
