The HBPC RN - Case Manager is directly responsible and accountable for the provision of quality nursing care to a culturally diverse veteran patient population. The incumbent possesses the knowledge and skills to utilize the nursing process in a collaborative, interdisciplinary practice setting to implement health promotion practices, manage acute and chronic illnesses, facilitate attainment of optimal levels of functioning, and to support the dying patient and his significant others Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
