OverviewSt. Luke's Health System in Meridian is seeking a Care Coordinator to join our NICU team. As a patient care coordinator, you will work collaboratively with the entire care team - both inpatient and outpatient - to facilitate safe transition from the hospital setting to home for NICU patients and families. We coordinate a variety of discharge needs ranging from equipment delivery, educating patients and/or family members, completing medication prior authorizations and collaborating with home health agencies in the community to name just a few.
