Overview
Atlantic Health System is seeking Per diem Inpatient RN Care Manager to work 8am to 4pm in our Case Management department at Chilton Medical Center including some weekends.
Responsibilities
Essential Functions:
Coordinating and facilitating the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
Clinical resource management through daily chart review, and Utilization reviews, coordination of discharge planning activities and comprehensive clinical documentation in the electronic medical record.
Participation in interdisciplinary patient care rounds, identifies and works to resolve quality of care issues, barriers to the progression of care and discharge delays.
Formulates, coordinates and implements discharge plans and an efficient plan of care to ensure safe, timely and effective discharge plan.

Minimum Education:
BSN required
License/Certifications:
Current NJ state RN License
Certified Case Manager preferred
Relevant Experience:
Hospital experience in case management a must, ability to manage multiple cases simultaneously, compassionate service delivery, excellent phone, fax and computer skills, willingness to work on a multidisciplinary team, time management and written documentation skills including legible hand writing
Experience with EPIC and CareWeb QI preferred
