The role you'll contribute:
Performs patient observation, data collection related to patient care, administers medication, treatment, and procedures, implements the plan of nursing care, and communicates findings to the supervising RN. Assumes responsibility and accountability for facilitating, communicating, and collaborating with the healthcare team to identify and meet the physical, emotional, and spiritual needs of the patients. Promotes the optimal health, well-being, and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines, and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care, organizational mission, and core values.
The value you'll bring to the team:
Reassess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records and clinical data collection. Evaluates patient’s response and intervenes to ensure optimal patient outcome. Reports appropriate patient findings to the supervising RN.
Contribute to/and implement patient plan of care and observe outcomes in accordance with standards and in collaboration with the interdisciplinary team.
Demonstrate competency in a variety of therapeutic/diagnostic interventions including IV’s, incision/wound care, medication administration, catheterization, and specimen.
Promote, advocate, and collaborate to protect the health, safety, and rights of each patient.
Provide patient/family education through reassessment of patient/family learning readiness, needs, and ability. Provide teaching and evaluate effectiveness of teaching.
Recognize the patient or designee as a full partner in providing compassionate and coordinated care, with respect for the patient’s preferences, values, and needs.
Qualifications
The expertise and experiences you’ll need to succeed:
Required:
High School Diploma/ GED- Required
Knowledge in nursing and person-centered care concepts.
LPN License to practice in Colorado.
Current Basic Life Support (BLS) Source: American Heart Association.
Preferred:
IV Therapy
Current state IV certification up hire or within three months of hire/transfer.
Previous patient care experience
