Frederick Health Hospice has partnered with Adventist Health Care to serve as their hospice provider of choice. Frederick Health Hospice will provide General Inpatient Patient hospice services at Shady Grove Adventist Medical Center and provide hospice services to clinically appropriate patient who are discharged from the hospital to receive hospice services at home. Home hospice services will also be provided in Long Term Care Nursing facilities and Assisted Living Communities where patient may reside.
For more than 43 years, Frederick Health Hospice has provided hospice care to both the Frederick and Montgomery County residents and their families. While until recently only serving resident in northern Montgomery County, this new partnership provides an opportunity to expand the scope of care to cover all residents of Montgomery County from a satellite office in Rockville, adjacent to the Shady Grove campus.
Job Summary
Provides skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the State Board of Nursing and department policies. The RN/Case Manager coordinates the interdisciplinary care team for the patient and family to assure continuity of care. Services may be provided to neonate, infant, and child, adolescent, adult and geriatric populations.
Required Knowledge, Skills and Abilities:
Demonstrates clinical competency in area of practice. Understands Hospice philosophy, principles of death/dying.
Must be licensed driver with a reliable vehicle that is insured in accordance with state and/or organizational requirements.
Self-directed with the ability to work with little supervision.
Ability to work effectively within an interdisciplinary team.
Demonstrates knowledge of and proper use of equipment listed on the Unit Specific Competency Tool.
Demonstrates knowledge of office equipment including but not limited to: telephone, photocopier, facsimile equipment, computer and printer.
Minimum Education, Training, and Experience Required:
Currently licensed as a Registered Nurse in the State of Maryland or Compact State.
Possesses and maintains current CPR certification.
At least 2 years general nursing experience is required with current Hospice, medical/surgical, critical care or home health experience. Experience providing pain management, IV skills preferred.
Bachelors of Science in Nursing Required or Associates degree in Nursing with a signed BSN agreement to obtain a BSN within 5 yrs of employment.
