Position Details:
Location: Reliant Medical Group, a Part of Optum: 900 Union St. Westborough, MA
Schedule: 4.5 days/week (Half-day TBD). Office hours are Monday - Friday; 8:00 am-5:30 pm. No evenings, weekends, or major holidays
Specialty: Adult Internal Medicine
Primary Responsibilities:
Determines the appropriate action to be taken involving decision making according to standard criteria
Communicates with providers and patients systematically via EMR or telephonically
Maintains tracking of patient lists, outreach and follow up
Offers clinician and patient support and/or education on workflows and protocols in the identified registry
Works with assigned supervisor/manager and director to develop, implement, and monitor procedures that support organizational goals and business objectives related to the Registry program

Required Qualifications:
Graduate of an accredited School of Nursing
Currently licensed as an RN/LPN in the State of Massachusetts
Basic Life Support (BLS)/CPR Certification
Preferred Qualifications:
Knowledge of EMR’s such as Epic
1+ years of hands-on clinical experience
Physical Requirement:
Physical health sufficient to meet the ergonomic standards and demands of the position
Unless certification, licensure or registration is required, an equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed in this description, may be substituted for the above requirements.
