Job DescriptionOpportunity open for a KY licensed Social Worker (LCSW, LPC, LPCC, LMFT) to join our health plan in the state of Kentucky. Applicants should have experience working with persons with serious emotional disturbances, or severe and persistent mental health conditions, including substance abuse disorders.  Most of the case management responsibilities will be handled telephonically, but there may be travel to the office in Louisville or to a community from time-to-time. The opportunity is open to all qualified applicants currently residing in the state of Kentucky or who are willing to relocate there. Preference will be given to those applicants who have previous experience working as a case manager/discharge planner within a managed care organization (
