Providence TrinityCare Hospice is seeking a Pediatric Palliative Care Nurse Practitioner to join their team covering the The San Fernando Valley and some of Los Angeles, CA. In this independent role, you will conduct high-level assessments, prescribe medications, and create care plans. You'll work as part of a collaborative, multidisciplinary team, providing in-home and telehealth care to pediatric patients. The focus will be on symptom management and aligning care with patient goals, all within the specialty of Palliative Care.
